Program Director – Supported Scatter Site Housing Program for People with Mental Illness
Overview: Comunilife is one of NYC’s major providers of supported scatter site units for people with mental illness. We serve formerly homeless people with serious and persistent mental illness (SPMIs) and other significant medical and substance abuse issues – providing these residents with well-maintained, individual apartments and a full range of care coordination and supportive services. The vast majority of program residents achieve long-term housing stability; many remain in our units for ten years or more – paying rent on time and in full. A significant segment of these residents obtain and maintain paid employment, and a small but consistent percentage move on to independent housing.
Responsibilities: The Program Director of Comunilife’s Supported Scatter Site Housing Program for People with Mental Illness is responsible for the planning, development, and monitoring and day-to-day operations of 280 units of housing in the Bronx and Queens. S/he directly supervises the Assistant Program Director for Queens Housing Units and the Clinical Supervisor for the Bronx Housing Units, who – in turn – supervise care coordination and supportive services.

Nurse Practitioner for Mi Salud
The Nurse Practitioner will be responsible for developing and implementing an integrated geriatric healthcare program for patients enrolled in Comunilife’s Vida Guidance Center. The program’s goal is to improve the health and wellness of seniors with mental illness and other behavioral health issues. Duties will include:
- Develop protocols to integrate physical health activities into a mental health setting.
- Conduct initial assessments with Vida Guidance Center patients and formulate health goals to be integrated into the comprehensive treatment plan and treatment plan reviews for each participating client. Make referrals to specialists as necessary
- Collaborate with other providers and coordinate care for participating clients
- Develop strategies to educate mental health staff on the need for and goals of the integrated healthcare program
- Develop weekly group sessions in diabetes, hypertension, smoking cessation and other behavioral health issues.
- Conduct monthly follow-up appointments with patients to chart their medical status, including pharmaceutical adherence and behavioral health changes.

Qualifications

Intake Coordinator
Responsibilities:
- Complete brief telephone assessment to determine appropriateness of referral to the clinic; perform initial screening for risk assessments.

Responsibilities:

Overview: Comunilife is one of the city’s major providers of transitional congregate housing for people with serious and persistent mental illnesses (SPMIs) and related behavioral and health issues. The screening, counseling, guidance, and linkage work provided by the social workers in these programs represents a critical component in an overall strategy that successfully prepared individuals with long histories of institutional care to move towards long-term housing stability.
Responsibilities: As a key member of the program team, the Social Worker will conduct psychiatric and other assessments and develop and implement individual-centered treatment plans, and supportive services for formerly homeless SPMI residents with co-occurring substance abuse disorders and/chronic health conditions. The services provided by this staff member will include providing individual, group and family counseling; assisting clients to achieve and maintain functional improvements in self-care, employment, personal relationships, and housing stability; and providing discharge planning and services.
Qualifications: Candidates must have an MSW, and at least three years prior experience working with persons who are formerly homeless, and who are diagnosed with mental illness and/or HIV/AIDS and other chronic health conditions. They must have experience providing individual and group counseling; and be able to participate in program evaluation activities. Overview: The ongoing, timely, responsible maintenance of physical plant is a vital component in assuring the success of Comunilife’s housing programs. By living in well-maintained and clean apartments and buildings, our program residents learn that their living environment is vitally important to us; and that their concerns about that environment are will be heard and addressed. As a result, they are similarly motivated to treat their surroundings with care and respect.
Responsibilities: The Maintenance Specialist is responsible for the overall cleanliness, repair, and safety of our congregate, supported and transitional housing, scatter site programs and office spaces. In particular, s/he is responsible for:
- Moving client furniture and personal effects as required.
- General mechanical repairs.
- Apartment heating and cooling systems.
- Painting and minor construction tasks (e.g., dry walling, patching and plugging holes).
- Attending to grounds; including snow and debris removal.
- Light plumbing (e.g., unclogging drains, replacing washers, plugging leaks).
- Light electrical repairs including changing bulbs and replacing switches.
- Lock replacement and repair.
- Tile and wood floor replacement, as needed.
- Carpentry including interior renovations and ductwork.

Case Management/Clinical Supervisor (CM/Clinical Supervisor)
The Case Management/Clinical Supervisor is responsible for the supervision and training of the care manager/mental health counselor, case manager/substance abuse, health educator and recreation therapist and entitlement specialist. Responsible for the delivery of comprehensive case management services and the coordination of the delivery of social support services to all clients. The CM/Clinical Supervisor is responsible for the assessment(s) of new clients; monitoring and ensuring program compliance with Comunilife’s standards of care; funding and regulatory entities. Assist the Assistant Program Director in the daily administration of the program.
QUALIFICATIONS:
Licensed Masters in Social Work (LMSW, or NYS CSW). At least two (2) years to five (5) years supervisory experience, overseeing a care management, substance abuse counseling team. S/he will be responsible for: crisis intervention; assistance in formulation and achievement of clients’ mental health goals and objectives; monitoring of medications; facilitation of group work on mental illness, medications management and other topics of relevance; advocacy around entitlements; coaching on activities of daily living and independent living skills; and linkages to the service providers who will continue supporting independence as residents move into more permanent housing situations.
Qualifications: Candidates should have a BA in a relevant major (e.g., psychology, health) preferred. A minimum of 1-2 years relevant experience assisting persons with mental illness; and strong interpersonal and communication skills (verbal and written) are required. Bi-lingual: English/ Spanish skills are a major plus.

Security Guards- Bronx, Brooklyn and Queens
RESPONSIBILITIES:
The Security Guard is responsible for the safety and support of the residence staff and clients afflicted with HIV+/AIDS, severe and persistent mental illness, and substance abuse individuals. The job includes ensuring safety of the staff’s working conditions, safety of the residents and facility. Required to cover nights and weekends, as needed.
QUALIFICATIONS:
High School diploma and 8 and 16 hour Security Guard Training Certificate with 1-2 years of experience in security, or healthcare field required. Experience with HIV/AIDS affected individuals, chronic mental illness and substance abuse individuals are a plus; New York State Security Guard License to be obtained within 6 months of employment. Bilingual, English/Spanish is preferred.
